Near Drowing
Protocols
Clinical Features:
- Hypothermia ,Tachycardia / bradycardia
- Tachypnoea,dyspnoea(pumonary edema)
- Altered level of consciousness
- Acidotic breathing (metabolic acidosis)
- Apnoea
- Renal failure
Investigations:
- CBC
- Coagulation profile
- ABG :Acidosis
- RBS:low
- SGPT & SGOT
- Serum electrolyte : Hypo or hypernatremia ,hyperkalemia
- Serum Creatinine
- Chest X-ray P/A view
- ECG
Rx:
At community level:
- Basic Life support
- Rescue pt from water
- Fingure-Sweep Maneuver
- CRP- If necessary
At Hospital :
- 02 inhallation - high flow (4L/min)
- Left Lateral position
- Inf. Normal Saline/Dextrosal mini
- Keep the Pt warm by Warm saline
If convulsion:
1. Inj Barbit(Phenobarbital) (200mg/1ml +9 ml D/W)
- LD:wt x 1ml I/V-stat &
- Then wt/4 ml IV--BD
2. Inj.Ceftron(Ceftriaxone)(500mg/5ml)
- wt/2 ml I/V stat...BD
3. Inj. Roxadex(Oradexon)(5mg/ml)
- wt x 0.15 ml.... I/V...QDS
Prognosis:
- ৪-৬ মিনিট এর বেশি পানিতে ডুবে থাকলে irresible brain damage হবে।
- বাচ্চা এলার্ট থাকলে ফুল রিকভারি হয়ে যায়।
Prevention:
- পুকুরের চারদিকে উচু দেওয়াল দিতে হবে।
- বাবা মাকে সব সময় সতর্ক থাকতে হবে বিশেষ করে বর্ষা কালে।
