Protocols
Type of Apnea:
1. Obstructive_sleep_apnea- এটা most common .এতে respiratory airway তে blockage থাকে।
Causes:
- Allergies
- Swollen adenoids/tonsil
- Overbite
- Overweight
- Hormonal changes at night
2. Central_sleep_apnea. এটা CNS অর্থাৎ brain এর সমস্যা l দেখা যায় 10-30 sec এর জন্য brain কোনাে respiratory signal দিতে পারে না।
mechanism :
Hyperapnea(hyperventrilation)➡Hypocapne a(কারন বেশি বেশি ventrilation হলে বেশি বেশি CO2 বের হয়ে যাবে)Apnea(আমরা জানি CO2 respiratory center কে stimulate করে। সুতরাং CO2 নেই respuration ও নেই) ➡Hypercapnea➡Нурerapnea এই cycle চলতেই থাকে।
3⃣Complex_sleep_apnea_syndrome এক সাথে patient এর obstructive এবং central sleep apnea দুইটাই থাকে।
S/S of Sleeping Disorder :
- Loud snoring
- Gasping for air during sleep
- Awakening with a dry mouth
- Morning headache
- Insomnia
- Daytime somnolence
- Difficulty paying attentio while awake
- Irritability
Risk Factor:
- Excessive weight
- Older age
- Sex:male
- Positive family history
- Smoking
- Alcohol,sedatives
some medical condition -
- High blood pressure
- DM type 2
- Congestive heart failure
- Prior stroke
- Chronic lung disease etc
Rx:
1. Budicort Nasal spray(Budesonide)
- 1 spray each nostril—BD---15 days
2. Tab. Deslor (desloradin)(5mg)
- 0+0+1......14 days
3. Tab. Esonix(esomeprazol) (20mg)
- 1+0+1........ 10 days (30 min before meal)
4. Antazol(Xylometazoline ) N/D 0.1%
- 3 drops each nose 3 times daily .....7 days(৩ফোটা ২ নাকে ৩বার)
5. Tab.Monas(montelukast)(10mg)
- 0+0+1………...1 month
Advice :
- Maintain a healthy weight. Doctors commonly recommend people with sleep apnea to lose weight.
- Try yoga. Regular exercise can increase your energy level, strengthen your heart, and improve sleep apnea
- Alter your sleep position.
- Use a humidifier.
- Avoid alcohol and smoking.
- Use oral appliances.
