Episiotomy
Protocols
Indication:
- Breech
- Forceps/ Vaccum delivery
- Occiput posterior position
- Rigid perineum
- Scarred perineum
- Shoulder dystocia
Timing: At crowning of head during a contraction
Procedure:
- Clean the area with antiseptic.
- Infiltrate using 10ml of 0.5-1% lignocaine.
- Check at the incision site for effect of local anesthetics.
- Perform episiotomy at crowning
- Place 2 fingers between baby's head and perineum.
- Cut 3-4 cm of perineum in mediolateral direction.
- After delivery of the baby and placenta, carefully examine for extension of tears.
- Repair the episiotomy with polyglactin raptide 2-0 suture or chromic catgut.
- Close the vaginal mucosa with continuous suture starting 1 cm above the apex.
- Perinea muscles are approximated using interrupted sutures.
- Skin is closed with interrupted sutures.
Rx :
Tab. Lebac(Cephradin) 500mg,
- 1+1+1+1..........7 days
Tab. Flux (Flucloxacin)500mg
- 1+1+1+1........7 days
Tab. Napadol( Tamadol)
- 1+04+1.... (খাবার পর).. ৩-৫দিন। (ব্যাথা থাকলে)....10 days
Tab. Vasco(Ascorbic acid) 250mg,
- 1+0+1..........10 days
Syp. Avolac (Lactulose)
- ২চামচ করে ৩বার....পায়খানা কষা হলে।
Advice:
1.নিয়মিত ঔষধ খাবেন।
2. পায়খানা প্রস্রাবের পর সেলাইয়ের জায়গা পানি
3. পরিস্কার করে টিসু দিয়ে মুষে নিবেন।
4. প্রয়ােজনে Povisep solution দিয়ে পরিস্কার রাখতে পারে।
5. ৬ সপ্তাহ স্বামী সহবস করবেন না।
6. কোন সামস্যা হলে ডা. পরামর্শণ নিবেন।
