Pre-operative order:

1.Diet: NPO 

2. Inf. Hartman solution (1000ml)

• ……10-20 drop/min

3.Inj.Ceftriaxone (1gm)

• 1 vial I/V ….stat

4. Inj.Esomeprazole(40mg)

• 1 vial I/V stat

5.Inj. Viset(Timonium)(5mg/2ml)

• 1/2 ample I/V stat (if labour pain-after consult with surgeon)

6.Clean the operative area .

7.send the patient OT table 30 min before operation .

8. Counsel the patient party & written consent must be taken.

• With all aseptic precaution after proper painting & draping the abdomen was opened by pfannestial incision.A transverse incision was given on the Lower uterine segment .A Male/Female healthy baby was delivered .Placenta & the membrane were removed by CCT. After maintaing proper haemostasis ,counting instrument & Mop,abdomen was closed layer by layer.There was less bleeding & no per operative complication . Skin closed by Vicryl/poly sorb intra dermally/interrupted suture.

2.Inf.Hartman (1L) + Inf.5%DNS(1L) + Inf.5%DA (1L) ….I/V 30 drop/min.

3. Inj.Ceftriaxone (1gm) … 1 vial I/V ….BD 

4. Inj.Esomeprazole(40mg) …1 vial I/V ….BD

5.Inj.Filmet(metronidazole) (500mg/100ml)…1 bottle I/V….stat.& TDS

6.Inj.Ketorolac(Rolac) (30mg)….1 ample I/M OR I/V stat & TDS 

OR 

Inj.Anadol (100mg/2ml) ….1 ample I/M OR I/V stat & TDS

7.Nalbun-2 …1 ample I/M stat 

OR 

Inj.Pethidine(100mg/2ml)…..1 ample I/M stat

With 

Inj.Vergon (12.5mg/ml) …1 ample …I/M stat 

8.Voltaline suppository (50mg) ….1 stick Per rectal –SOS

9.Monitor all Vitals…any abnormality then call doctor

1.Diet : Sips of water then Liquid the Normal 

2. Ceftriaxone (1gm)/ceftron

• 1 vial I/V ….BD 

3.Tab.Metronidazol(Filmet)(400mg)

• 1+1+1( many surgeon use Clindamycin( 300mg) 1+0+1)

4.Cap.Esomeprazole(sergel) (20mg)

• 1+0+1…(30min before meal)

5.Tab.Ketorolac(Torax) (10mg)

• 1+0+1 …(after meal) OR Cap.anadol(50mg) ..1+0+1

6. Tab.Domperidone(omidon)(10mg)

• 2+2+2 OR 1+1+1 ( for milk secretion)

7.Tab.Ceevit (250mg)

• 1+0+1

8.Tab. Clonazepam ( Clon) (0.5mg)

• 0+0+1 

9.Voltaline suppository (50mg)

• 1 stick per rectal --SOS

Discharge of LUCS :

1. Cap.Denver (200 mg)/Cefixime

• 1+0+1........7 days
  

2. Cap.Esonix (20 mg)/Esomeprazole

• 1+0+1.......10 days( খাবার ৩০ মিনিট আগে)
  

3. Cap.Flustar (500 mg)/Flucloxacillin

• 1+1+1+1......7 days

OR

Cap. Qcin (300mg)/Clindamycin

• 1+0+1...........7 days

4.Tab.Rolac (10 mg)/Ketorolac

• 1+0+1....3 -5 days ( ভরা পেটে)

5.Tab.Ridon (10 mg)/Domperidone

• 2+2+2......10 days (দুধ না পেলে)
  

6.Cap.D-sefa

• 1+0+1…………...2 months
  

7.Tab.Rivo(0.5mg)/Clonazepam

• 0+0+1…...15 days
  

8.Tab.Ascoson(250mg)/Ascorbic acis

• 1+0+1….10 days
  

১৫ দিন পর থেকে(after 15 days)

Cap.Zif-CI/iron+zinc

• 0+0+1....1 month(after meal)
  

Tab.Calcin-0/calcium

• 0+1+0......3 months(after meal)
  

উপদেশ/Advice:

• ১) ৩মাস ভারী কাজ করবেন না। 
• ২) দেড় মাস স্বামী সহবাস করবেন না। 
• ৩) দেড় মাস পর থেকে জন্মনিরোধক পদ্ধতি ব্যবহার করবেন। 
• ৪) দেড় মাস বয়স থেকে বাচ্চা কে টিকা দিবেন। 
• ৫) ৬ মাস বয়স পর্যন্ত বাচ্চা কে শুধু মাত্র বুকের দুধ খাওয়াবেন। (ঔষুধ খাওয়ানো যাবে)
• ৬) আগামী........... তারিখে সেলাই কাটবেন।
