Protocols
Clinical Features:
- Patients lives edemic zone with
- High grade fever
- Anorexia
- Weight loss
- Pallor
- bleeding
- Abdominal distension
On examination:
- Pallor (+++)
- Jaundice
- petechae ,purpura: +
- Hepatomegaly
- Huge splennomegaly
Investigation:
- CBC With PBF
- low Hb
- leucopenia
- thrombocytopenia
- monocytosis
- RK-19 : positive
- ICT for kala azar
- Bone marrow: LD bodies
Rx:(National guideline)
1st line Drug:
Inj.Tericin(50mg/vial)(Liposomal Amphotericin-B)
- 10mg/kg.... I/V drip Single dose.
OR
Cap.Miltefos(50mg)(Miltefosine)
- 2.5 mg/kg/day....BD......28 days (After meal)
OR
Combination Treatment :
Day-01
Inj.Tericin(50mg/vial)(Liposomal Amphotericin-B)
- 10mg/kg.... I/V drip Single dose.
Day- (2-8)
Cap.Miltefos(50mg)(Miltefosine)
- 2.5 mg/kg/day....BD......28 days (after meal)
2nd line drugs:
- Pregnancy Safe drug also
Inj.Stibtin/Sodium Stibogluconate (100mg/ml)
- 20mg/kg..IM/IV...drip@ 10-20 drop/min... Daily for 30 days
If Fever:
1.Tab.Napa([post id="449" title="Paracetamol"])(500mg)
- 1+1+1....3-5 days
If 2ndry infection :
Tab.Cipro([post id="408" title="Ciprofloxacin"])(500mg)
- 1+0+1.......7 days
Tab.Seclo([post id="391" title="Omeprazole"])(20mg)
- 1+0+1.......15 days
