Dengue Fever

Clinical Features:

A) Febrile Phase:(2-7 days)

• Severe headache
• Retro-orbital pain
•  Severe myalgia/arthralgia/back pain
•  Anorexia, nausea, vomiting 
• Abdominal pain
• Photophobia
•  Altered taste sensation
•  Constipation 
•  Some may have -
•  Positive tourniquet test
•  Evidence of muco-
• cutaneous bleeding e.g. petechiae, 
• purpura, ecchymosis, gum and nose bleeds
•  Low platelet count

B): Critical phase:

• Plasma leakage 
• haematemesis, malaena. 
• Severe Bleeding 

Warning Sign:

• Abdominal pain or tenderness
•  Persistent vomiting
•  Clinical fluid accumulation e.g. ascites,effusion
•  Bleeding:
•  Petechiae or purpura
•  Bleeding from nose or gums
•  Vomiting of blood 
•  Heavy menstruation/vaginal bleeding
•  Lethargy, restlessness
•  Liver enlargement >2 cm
•  Increased HCT 
• Rapid decline in platelet count
•  Drowsiness, mental confusion or seizures
• Pale, cold or clammy hands and feet
•  Difficulty in breathing
•  Postural dizziness
•  No urine output for 4–6 hours

investigation :

• 1)Dengue NS1Ag, ( ১ম ৪ দিন) 
  
• 2)Dengue lgG, lgM (জ্বরের 6 দিন পর করতে দিবাে)
• 3)serum electrolyte 
• 4)LFT(SGPT,SGOT-incease)
• 5) RT-PCR 
• 6)Virus isolation 
• 7) PT, APTT -prolong
• 8)X-ray chest - see pleural effusion 
• 9)USG of W/A - to detect ascites
• 10)Complete Blood Count: CBC☑️
• TC (leukopenia). Leukopenia 👉 Platelet count: Reduced 
•  Haematocrit: Raised. A ≥20% rise in haematocrit from the baseline is indicative of significant plasma leakage

Rx:

Group-A: Dengue without warning sign:

(Symptomatic&supportive) 

• More Fluid

1. Tab. Napa (500mg)

• 2+2+2 (জ্বর >100' f হলে ভরাপেটে খাবেন)

2.Supp. Napa (500mg) ১ টা পায়ু পথে (জ্বর > ১০২ F হলে)

3.Tab. Oradin (10mg) /Fenadin (120mg)/Alatrol (10mg)

• 0+0+1 (সর্দি থাকলে).....7 days

Group-B: Dengue with Warning sign (No shock)-DHF with plasma leakage :

• Hospitalization. 
  

💉inf.Normal saline ( 1.5 ml/kg/hour.... Next 6 hour)

⬇️If stable then........ Oral fluid 

⬇️If No improvement 

• 3-5ml/kg/hour... For next 6 hour 

⬇️If stable then maintain 48 hours 

• Reduce fluid 5>3>1.5 ml/kg/hr..... Then oral 

F/U: 1)Vital sign, 2)Hct, 3)Urine out

Group-B : Dengue with warning sign with shock(DSS): 

• Manage in ICU 

Prevention:

• Protect yourself against mosquito bites.
  
• Prevent mosquito breeding inside and outside your home.
  
• Avoid visiting areas prone to mosquitoes.
  

কখন হাসপাতালে ভর্তি হবে।

• শরীরের যেকোনো অংশে রক্তপাত হলে
  
•  প্লাটিলেটের মাত্রা কমে গেলে
  
• শ্বাসকষ্ট হলে বা পেট ফুলে পানি এলে
  
•  প্রস্রাবের পরিমাণ কমে গেলে
  
•  জন্ডিস দেখা দিলে
  
• অতিরিক্ত ক্লান্তি বা দুর্বলতা দেখা দিলে
  
• প্রচণ্ড পেটে ব্যথা বা বমি হলে।
