Idiopathic Thrombocytopenic Purpura(ITP)

Protocols

Clinical Features:

  • 1)Easy or excessive bruising.
  • 2)Superficial bleeding into the skin that appears as pinpoint-sized reddish-purple spots (petechiae) that look like a rash, usually on the lower legs.
  • 3)Bleeding from the gums or nose.
  • 4)Blood in urine or stools.
  • 5)Unusually heavy menstrual flow.


O/E: 

  • Multiple petechiae & purpura over whole body with color Change. 

Investigations: 

  • 1)CBC with PBF : thrombocytopenia
  • 2) Urine R/M/E: exclude HSP
  • 3)PT
  • 4)APTT 
  • 5) ANA - vasculitis 

Rx: 

1.Tab.cortan(prednisolone) 20 mg 

  • 3+0+0.........1 months
  • 2.5 +0+0......15 days
  • 2+০+০..........15 days
  • 1+০+০...........15 days 
  • ১/২ +০+০.......15 days

Then

Tab.cortan(prednisolone) 5 mg 

  • 1+0+0.......১৫ দিন 

Then,

Every alternative day Tab.Cortan(5mg) ......15 days

(১ টি tab.cortan 5 mg একদিন পর পর....১৫ দিন )


2.Cap.Sergel (20mg) 

  • 1+0+1.......3 months( খাবারের আগে যতদিন cortan চলবে) 


3.Tab.Rocal-D 

  • 1+0+1..........4 months

[Note : আমরা cortan এর পরিবর্তনে other drug use করতে পারি 

4.Tab.Eltrom(eltrombopag) 50mg

  • 1+0+0.....3 months
