Hemophilia

Clinical Features:

• 1)Unexplained and excessive bleeding from cuts or injuries, or after surgery or dental work.
• 2)Many large or deep bruises.
• 3)Unusual bleeding after vaccinations.
• 4)Pain, swelling or tightness in your joints.
• 5)Blood in your urine or stool.
• 6)Nosebleeds without a known cause.
• 7)In infants, bleeding during crawling tooth extraction or circumcision. 

Note : on history তে মায়ের দিকের মামাদের কারো এই রোগ থাকবে। Female carrier & male affected হয়। 

O/E : 

• prolong bleeding 
• Tender swelling knee or elbow joints 

Investigations: 

• 1)CBC with PBF 
  
• 2)PT - normal 
• 3) APTT- prolong
• 4) Factor VIII & IX assy
• 👉FVIII deficiency - Hemophilia A
• 👉FIX deficiency - Hemophilia B 
• 5) BT - normal 

Rx: (hospital treatment) 

Inj.FVIII or FIX for 3-5 days 

OR

inj.Fresh frozen plasma(FFP) for 3-5 days 

if severe bleeding 

 inj. Traxyl 

• 1 amp i/v stat & TDS.... 5-7 days 

in any infection 

inj.Ceftriaxone (1g)

• 1 vial i/v stat & BD..... 7 days 

Advice: 

• পরবর্তীতে হিমোফিলিয়া ক্লিনিক বা সোসাইটি তে যোগাযোগ রাখবেন। 
  
• আগাত বা মারামারি কাজ থেকে দূরে থাকবেন।
