Acute Leukemia

Clinical Features:

• 1)Bleeding from the gums.
• 2)Bone pain.
• 3)Fever.
• 4)Frequent infections.
• 5)Frequent or severe nosebleeds.
• 6)Lumps caused by swollen lymph nodes in and around the neck, armpits, abdomen or groin.
• 7)Pale skin.
• 8)Shortness of breath.

O/E:

• Anaemia- +++
  
• Bony tenderness:. ++
• Ghum bleeding present 
• Tem- 101 or more 
• Hepatosplenomegally

Investigations: 

• 1) CBC with PBF 
  
• 2)Blood grouping 
• 3) liver function test (LFT)
• 4) Renal function test (RFT)
• 5)Blood c/s 
• 6)Urine c/s 
• 7) serum uric acid 
• 8)Bone marrow study 

Rx: ( Hospital treatment) 

• (leukemia pt এর infection হবার chance অনেক বেশি তাই antibiotics coverage রাখতে হবে)

1. Inf.Normal saline 

2.Inj.ceftriaxone (1gm)

• I/v stat & BD

3.Inj.amkacin(Kacin) 

• 1 amp i/v stat & 12 hly 

4.Tab.Maxpro(20 mg)

• 1+0+1......15 days

5.Tab.Virux (Acyclovir)(400mg)

• 1+1+1+1+1....10 days

6.cap.Flugal(Fluconazole) 150 mg 

• 0+0+1......10 days

• Maintain tem & vitals 
• Maintain proper oral hygiene 
  
• Care of natural orifice 
  

if Hb কম তাহলে blood or blood products দিতে হবে। 

Follow up: একজন hemato-oncologist এর কাছে পাঠাতে হবে।
