Hyperkalaemia
Protocols
Hyperkalaemia:
Mild Hyperkalaemia : (>5.5➖6 mmol /L)
- Diet: Fruits/Drugs Restriction
Moderate to severe Hyperkalaemia :(> 6 mmol/L)
1️⃣ Myocardial cell membrane stabilization :
☯️Calcium gluconate (10%)
- Dose: 0.5-1 ml/kg, I/V slowly over 5-10 minutes.
2️⃣Redistribution of extracellular K+ into cell:
☯️Insulin:(short acting)
- Dose: 0.1 unit/kg, I/V with 10% DA @ 5 ml/kg over 30 minutes.
☯️Salbutamol Nebulization :
- Dose: 2.5mg( less then 25kg) or 5 mg(>25kg)with Normal saline(2 ml)
☯️Sodium Bicarbonate:
- Dose: 1-2 mmol/kg,I/V over 10-15 min
3️⃣Enhance Elimination of K+ from the body through gut:
☯️Sodium polystyrene sulfonate resin(kayexalate):
- Oral: 1month -18 years
- Dose: 125-250mg/kg(Max.15g)in 15-30ml 70% sorbitol over 3-4/ days
Rectal : Neonate
- Dose: 125-250mg/kg, dilute eachbgm resin in 5-10ml methylcellulose or water, repeated as necessary, every 6-8 hrly.
