Meningitis/Encephalitis

Protocols

Clinical Features:
  • 1)high temperature.(menigitis)
  • 2)cold hands and feet.
  • 3)vomiting.
  • 4)confusion.
  • 5)breathing quickly.
  • 6)muscle and joint pain.
  • 7)pale, mottled or blotchy skin.
  • 8)spots or a rash.


 3 CARDINAL sign of meningitis:

  • 1) Neck rigidity
  • 2)Kernig sign positive
  • 3)Brudzinski's sign positive

Encephalitis:

  • Short history 

INVESTIGATIONS:

  • 1)CBC 
  • 2)RBS
  • 3)CSF STUDY
  • 4)CT SCAN
Rx:
  • Bed Rest
  • 02 Inhallation sos
  • Continous Catheterization 
  • Change posture 2 hrly
  • NG Feeding = 200ml - 2 hourly (8am-10pm)

1. Inf. Normal Saline (1L) IV 10 d/min

2. Inj.Oricef([post id="398" title="Ceftriaxone"])(1gm)...1 vial I/V stat & BD

3.  Inj.Maxpro([post id="392" title="Esomeprazole"]) (40mg)...1apm I/V stat & BD

4. Inj. Oradexon 1 amp IV stat & 6 hourly

6. Supp. Napa([post id="449" title="Paracetamol"]) (500mg) 1 stick P/R if temp > 102'F

If we suspect encephalitis:
  •  Inj. Virux([post id="612" title="Acyclovir"]) (500mg) 1vial I/V TDS/QDS

🚏If convulsion: 

  • Inj. Sedil([post id="476" title="Diazepam"]) 1amp I/M stat & sos

OR

🚏 If Bacterial Meningitis:
  • Inj.[post id="437" title="Vancomycin"]/Covan(1gm)...1 vial Dissolved with 200ml N/S over 1 hour....BD 

Discharge of Meningitis:

1.Cap.Denver([post id="399" title="Cefixime"])(400mg)..1+0+1....7 days১+০+১....৭ দিন 

2.Cap.Maxpro([post id="392" title="Esomeprazole"])((20mg) 1+0+1(30min before meal)...15 days

3.Tab.Cortan(20mg)...1+0+0....7 days

4.Tab.Barbit(30mg)...0+0+1....15 days
