Hyperthyroidism

Clinical Features:

• 1)nervousness, anxiety and irritability.
• 2)mood swings.
• 3)difficulty sleeping.
• 4)persistent tiredness and weakness.
• 5)sensitivity to heat.
• 6)swelling in your neck from an enlarged thyroid gland (goitre)
• 7)an irregular and/or unusually fast heart rate (palpitations)
• 8)twitching or trembling.

O/E:

• 1)tachycardia
• 2)tremor
• 3)systolic hypertension
• 4)gpitre
• 5)warm & moist skin
• 6)lid retraction & gynecomastia 

investigation:

• 1)serum T3 & T4-increase
• 2)Seum TSH -decrease
• 3)ECG-

Rx:

1. Tab. Mercazole(Carbimazole) (5mg)

3+3+3 (after meal)..…...3 wks 

Then, 2+2+2....(after meal)..….6 wks

Then, 1+1+1.....(after meal )..…..continue

🚏if Palpitation:

• Tab. Indever(Propranolol ) (40mg) 1/2+০+1/2.…..3 wks
  

Then, 

• Tab.Indever(Propranolol )(10mg)...1+০+1.…...continue

🚏উপদেশ: ৬ সপ্তাহ পর পুনরায় S.TSH,ST4 পরীক্ষা করবেন। এবং

প্রয়ােজনমত Drug এর ডােজ বাড়বে/কমবে।
