Chronic Renal Failur(CRF)

Clinical Features:

• 1)Nausea and vomiting, 
• 2)muscle cramps, 
• 3)loss of appetite, 
• 4)swelling via feet and ankles, 
• 5)dry, itchy skin, 
• 6)shortness of breath, 
• 7)trouble sleeping, 
• 8)urinating either too much or too little.

Stage of CKD:

Investigations:

• 1)Urine RME-proteinuria
• 2)CBC& PBF-Normocytic normochromic anaemia
• 3)Serum Creatinine-raises
• 4)BUN-raised
• 5)X-ray of KUB
• 6)USG KUB
• Bilateral shrunken echogenic kidneys/hydronephrosis
• 7)Serum electrolyte
• Hyperkalemia
• hyponatremia
• 8)lipid profile 
• Hyperlipidaemia
• 9)DTPA/DMSA Scan
• Renal scarring/impaired renal excretion
• 10)MCU/VCUG 
• To see posterior urethral valve 
• 11)IVU 
• To see VUR,urinary tract,status of kidney

Rx:

• Diet – Fluid – Protein normal (60 mg /day ) 
  

Correction of anaemia ( 10 to 12 gm /dl) 

• Blood transfusion
  
• Erythropoitin ( in rich patient ) Other 
  
• Tab. Feofol.... 1 + 0 + 1
  

For hypocalcaemia 

• Tab. Dicaltrol (calcitriol) ( 0.25µmg ) 
• 0 + 0 + 1....continue

For binding dietary PO4

• Tab. Calbo(calcium) (500mg )
  
• 1 + 1 + 1 

PPI 

• Tab. Pantonix(pantoprazole) (20 mg )
  
• 1 + 0 + 1 

Anti HTN if needed (single or combination) 

• Tab. Angilock 50mg / camlodin 5mg / alphapress 1mg
• 0 + 0 +1 
  

If edema / decrease out put 

• Tab. Fusid 40mg
• 1+1+1 / 1 + 1+0 

If vomiting 

• Tab. Omidon 10 mg
• 1 + 1+ 1 
  

If infection non nephrotoxic anti biotic 

• Inj.ceftriaxone / amoxycilin /azithromycin
  

উপদেশঃ 

• আমিষ জাতীয় খাবার 60 mg এর বেশি খাবেন না। 
  
• ধুমপান করবেন না।
  
• চর্বি জাতীয় খাবার খাবেন না। 
  
• পাতে লবন খাবেন না।
  
•  ফলের রস, ডাব, কলা খাবেন না।
  
• নিয়মিত শারীরিক ব্যয়াম করুন।

Indication of dial sis in CRF WHO criteria 

• 1. Uraemic pericarditis
• 2. Uraemic encephalopathy or neuropathy 
• 3. Pulmonary edema unresponsive to diuretics. 
• 4. Severe hypertension if not control by medical management 
• 5. Severe hyperknlacmia not control by medical management 
• 6. Severe bleeding diathesis. 
• 7. S. Creatinine more then12 mg/dl or BUN more then 100mg/dl
