Sleeping Disorder /Apnea
♈ Type of Apnea
1⃣Obstructive_sleep_apnea- এটা most
common .এতে respiratory airway তে
blockage থাকে।
কারনঃCause
- Allergies
- Swollen adenoids/tonsil
- Overbite
- Overweight
- Hormonal changes at night
2⃣Central_sleep_apnea.
এটা CNS অর্থাৎ brain
এর সমস্যা l দেখা যায় 10-30 sec এর জন্য brain কোনাে respiratory signal দিতে পারে না।
mechanism ঃ
Hyperapnea(hyperventrilation)➡Hypocapne
a(কারন বেশি বেশি ventrilation হলে বেশি বেশি CO2
বের হয়ে যাবে)Apnea(আমরা জানি CO2
respiratory center কে stimulate করে। সুতরাং CO2 নেই respuration ও নেই) ➡Hypercapnea➡Нурerapnea
এই cycle চলতেই থাকে।
3⃣Complex_sleep_apnea_syndrome এক সাথে
patient এর obstructive এবং central sleep
apnea দুইটাই থাকে।
♈S/S of Sleeping Disorder :
- Loud snoring
- Gasping for air during sleep
- Awakening with a dry mouth
- Morning headache
- Insomnia
- Daytime somnolence
- Difficulty paying attentio while awake
- Irritability
♈Risk Factor:
- Excessive weight
- Older age
- Sex:male
- Positive family history
- Smoking
- Alcohol,sedatives
কিছু medical condition -
- High blood pressure
- DM type 2
- Congestive heart failure
- Prior stroke
- Chronic lung disease etc
Rx:
1. প্রথমে patient কে sleeping pill, alcohol
এগুলাে use করা বন্ধ করতে বলতে হবে, কেননা এগুলাে
throat muscle relax করে airway narrow করে
দিতে পারে।
🖍Sleep on side (এত airway তে
pressure কম পরে)
🖍Continuous positive airway pressure
device:( positive pressure এর ফলে airway
narrow হতে পারে না)
🖍Use Oral Mouthpiece
Drug Treatment :
1. Budicort Nasal spray 1 spray each nostril—BD---15 days
2. Tab. Deslor([post id="834" title="Desloratadine"]) (5mg) o+o+১......১৪ দিন
3.Tab. Esonix([post id="392" title="Esomeprazole"]) (20mg) ১+০+১....(খাবার আগে)...৭দিন
4. Antazol N/D 0.1% ৩ফোটা ২ নাকে ৩বার...৭দিন
5. Tab.Monas ([post id="720" title="Montelukast"])(10mg) 1………...1 month
