Clinical Features:
1.Wheal ,raised itchy lesion
2. H/O short duration
3. H/O taking allergic food .
Investigations:
1. CBC with ESR — Raised eosinophils count, raised ESR
2. Total eosinophils count, IgE - Increased
3. Urine R/ME: UTI
4. Stool R/M/E ; Ova or parasite
5. FT4, TSH
6. ANA, Anti dsDNA -
7.Ice cube test — Cold urticaria
8. Skin biopsy — In Chronic urticaria
D/D:
- Vasculitis
- Drug Reaction
🗒️Rx:
Acute :
1.Tab.Xyril ([post id="534" title="Hydroxyzine"])(10 mg) ... 1+1+1....১৫ দিন
2.Tab.Famotack ([post id="395" title="Famotidine"])(20mg)...1+0+1(before meal).....10 days
3.Tab.Cortan([post id="479" title="Prednisolone"])(10/20mg)....1+0+1......(after meal)..3/5 days
4.Togent ointment([post id="611" title="Diphenhydramine"])...3 times daily ...7 days
5. Tab.Alben Ds([post id="536" title="Albendazole"]) (400mg) ১ টা ট্যাবলেট ১ম ও ৭ম দিন চুষে খাবেন
if need :
1.Inj.Avil ([post id="706" title="Pheniramine"]) ......1 amp I/M stat
2. Inj.Cotson/Roxadex([post id="472" title="Dexamethasone"]) ....1 vial l/V stat
Chronic:
5.Tan.Monas([post id="720" title="Montelukast"])(10mg)....0+0+1......1 month
6. Betaderm ointment([post id="539" title="Betamethasone"])
+ Calamine ([post id="532" title="Calamine"]) lotion ....2 times daily....1 month
Advice:
1.Avoid triggers.
2.Apply cold washcloth.
3.Take a comfortably cool bath.
4.Wear loose, smooth-textured cotton clothing.
5.Avoid the sun.
